Oral Evidence
Will the NHS never learn? Follow-up to PHSO report ‘Learning from Mistakes’ on the NHS in England
I provided oral evidence to a Committee meeting Chaired by Sir Bernard Jenkin MP in The Houses of Parliament, London, on Tuesday 8 November 2016.
This Report focuses on the issues arising from the Parliamentary and Health Service Ombudsman’s (PHSO) July 2016 report, ‘Learning from mistakes: An investigation report by the Parliamentary and Health Service Ombudsman into how the NHS in England failed to properly investigate the death of a three-year old child.’ Oral evidence was given in The Houses of Parliament, recorded and broadcast here. The Williams Review
I provided oral evidence to an evidence hearing Chaired by Sir Norman Williams FRCS FRCP FRCPE FMedSci at De Vere Grand Connaught Rooms, London, on 6 April 2018.
The Williams Review was rapid policy review into gross negligence manslaughter in healthcare was chaired by Professor Sir Norman Williams. The review was set up to make recommendations to support a more just and learning culture in the healthcare system. The report was published on 11 June 2018 and is available here.
